The formation of gallstones - PubMed

Brown stones are formed of calcium bilirubinate and are usually associated with biliary infection. They occur in both the gallbladder and the bile ducts. Black pigment stones are extremely hard bilirubin polymers and are found mainly in the gallbladder. Biliary sludge is a necessary precedent of gallstones.

In 15 of them, bile was found to be abnormal: cholesterol monohydrate crystals were seen in 11, cholesterol crystals + calcium bilirubinate granules in 2 and calcium bilirubinate granules in 2. To date, nine of these patients have been operated on: 6 (all with cholesterol monohydrate crystals) had small cholesterol gallstones and 3 (2 with

In 11 patients, bilirubinate granules were found, cholesterol microcrystals in two, and Giardia lalia in two. Conclusion:Bilirubinate granules are the main pathologic component of bile in patients with acute idiopathic pancreatitis. Cholecystectomy is the preferred therapeutic approach.

Calcium bilirubinate granules were found more frequently in men (nine men vs. four women, P less than 0.001). Of the 21 patients in whom biliary sludge was the only finding (2 patients also had dilasted bile ducts when restudied), the 6 treated by cholecystectomy and the 4 treated by papillotomy had fewer recurrences of acute pancreatitis

Microlithiasis: an important cause of ‘idiopathic’ acute

01/01/2003· The small irregular aggregates are calcium bilirubinate granules. Tumefactive sludge mimicking gallbladder neoplasm: …

Biliary sludge is a suspension of cholesterol monohydrate crystals and calcium bilirubinate granules in a matrix of mucous gel and bile. It is essentially diagnosed by ultrasonography, and it may play a pathogenic role in the etiology of biliary colic pain and acute pancreatitis. [3]

Primary bile duct stones and bacterial activity

Microscopic examination of bile showed abundant precipitation of calcium bilirubinate granules in all samples. Thus, bile duct bile infection with beta-glucuronidase active bacteria (e.g. E. coli, C. perfringens) appears to be a key factor in PBDS pathogenesis, having a precursor role, rather than being a …

01/03/1998· Presence of cholesterol microcrystals and calcium bilirubinate granules strongly suggests the ul- terior development of cholesterol and pigmentary stones, respectively (2). Calcium carbonate microspherolites can also be found, but their diagnostic value is poor (3). The aim of this study was to evaluate the main pathologic component of bile

The authors consider that the observation of LLE in the gallbladder bile is an important index of the presence of cholesterol crystals and/or calcium bilirubinate granules. These LLE can assume polymorphic aspects, described by the authors, which may simulate other pathologies and lead to incorrect echographic diagnosis.

Two types of deposits may be evident: cholesterol crystals and/or calcium bilirubinate granules. Cholesterol microcrystals are birifringent and rhooid shaped and best visualized by polarizing microscopy. The presence of cholesterol crystals provides a reasonably high diagnostic accuracy for microlithiasis. 13. Dill J.E.
